Consent

I hereby confirm that I consent to the performance of the (genetic) analysis on myself or my child, for whom I am the legal representative. This consent declaration can be revoked by me at any time; furthermore, I have the right to have any examination results that may already be available destroyed.

Medical Aspects

In the case of a medical analysis, I confirm with my signature that I have been sufficiently informed about the nature, scope, significance, and possible consequences of the planned (genetic) examination, and have had the opportunity to have any remaining questions answered. I have also been adequately informed about the course of a possible illness, as well as the probability of passing on any genetic predisposition to children. Parents or siblings may also be carriers of this genetic predisposition. I am also aware that the indicated health risks in a medical analysis are based on statistical probability and do not represent absolute disease facts. I also understand that the recommended preventive measures may reduce the risk of illness but cannot exclude the possibility of becoming ill. The examination results will not be shared with family members, their treating physicians, or third parties.

Assignment

I hereby instruct Novogenia GmbH laboratory to perform the technical analysis of the enclosed sample(s), and DNA Plus - Center for Human Genetics GmbH to evaluate the results and prepare the corresponding reports. The service provider is exclusively DNA Plus - Center for Human Genetics GmbH. I am aware of this source of information and have had sufficient time and opportunity to familiarize myself with it. I accept the current terms and conditions and agree to the terms. I have been informed that I am responsible for making a copy of the contract (analog or digital). Options include photographing the form with a smartphone or copying the form. If necessary, a copy of the contract can be requested from the laboratory at any time.

Procedure

I agree that the consultation provided by the laboratory will take place solely via modern means of communication and that the results will be generated, printed by an external printing company, and delivered by mail and/or email with consideration for data protection. I also agree that the sample may be sent to an external laboratory for control analysis in anonymized form if necessary. My data security will not be compromised. Furthermore, I confirm that in the case of capillary blood collection, the fingerstick was performed by myself.

Data Processing and Storage

I hereby confirm that the data provided by me may be processed and stored in accordance with the relevant legal provisions (GDPR, etc.). This applies in particular to Novogenia GmbH laboratory and DNA Plus - Center for Human Genetics GmbH, including their employees working within the scope of the assignment given by me. This consent also applies to the companies and individuals who have facilitated the analysis assignment. Furthermore, I expressly confirm having read, understood, and agreed to the privacy information at https://novogenia.com/agb/. Direct marketing and contact for marketing purposes I expressly consent (especially according to § 107 TelekommunikationsG, etc.) to be informed by Novogenia GmbH laboratory and DNA Plus - Center for Human Genetics GmbH about free vouchers for new tests, messages, and products. This consent can be revoked by me at any time.
